I went there about 20 years ago on an old passport and cannot remember the exact date to apply for new visa. What do I state in the application?

Can you find the old passport? If you can, the dates for your last travel can be found on the passport. If you can't find the passport, just fill in the rough dates and explain to the consular officer if being asked.
