I am Italian, I use a visa in Shanghai before going to the Philippines within 72 hours. After a week in the Philippines can I take the return flight to Shanghai for Italy?

Yes, you can. If your itinerary looks like:

Philippines - Shanghai - Italy

And you don't have any additional stop of any kind within mainland China, you are definitely eligible for 24 or 144 hour visa free transit in Shanghai area. This means that you are allowed to stay visa free in Shanghai area from a just few hours for a short transfer from one flight to the other to 144 hours. It will be your decision what option will you use, but you will have decide it before your departure from the Philippines and also you need a confirmed tickets to be allowed to do that. In reality, this is very simple. Just print out your flight e-tickets and this is it.
