1. Yes, that is OK and you should bring your old passport with your used Chinese visa inside, along with your new passport.
2. Exactly. Without the previously granted Chinese visa, your chances for VOA would be mildly speaking uncertain. It is practically always case by case basis in VOA approval, including for the citizens of the countries that are eligible for 144 hours TWOV. Sometimes they appear even on this forum complaining that this particular visa wasn't granted for some reason.
3. The best bet is to apply for your VOA at Luhou border crossing. You will first cross Lo Wu crossing that is Hong Kong counterpart to mainland Chinese Luohu crossing. You could also apply on some other crossings, but I would definitely suggest Luohu. It is also the most convenient transport wise.
