For example, US citizen travelling to Malaysia and Singapore on business - can this person apply for a China entry permit when he is in Kuala Lumpur or Singapore?

If you have valid residence permit or visa in there, it's possible for you to apply from the CVASC. If you are still worried, you can contact the visa office to confirm it.
