About:China Train

When booking, I am offered a place to include a 'surname' and a 'given name'

but my passport also includes my middle name. Do I need to include my middle name when booking, and if so, in which category- surname or given name?

If there is a middle name on passport, you need to put it in "given names" blank following your first name.
