Passport and Resident Permit Expiry, Missing Documents

I am a student studying in Guangzhou for 6 years, I started this October and will end on 2023. My residence permit will expire on July 16th and my passport will expire on July 17th.
So I am planning to go back to my country, Indonesia, January to February 2018 to renew my passport and then extend my residence permit after. The problem is when I applied for the university, I didn't get my JW202 form. Instead, I used another university's JW202 form to apply for the visa, and now the university is keeping the other university's JW202 form. And I've lost my admission letter somewhere (pretty sure I can get a replacement though).

So what should I do? Do I need to apply for a visa? And what are the requirements for extending a residence permit in China? Thank you.

If you want to extend a residence permit in China, you should prepare the passport and its copy, the application form, a passport-style photo, residence permit, certificate of physical examination, your admission notice and JW form as well as other documents the consular officers consider necessary.

Usually, the physical examination report is valid for 6 months, so if it is still valid then, you can use it.

Thank you for your answer. I've asked my teacher if I will need my admission notice and JW form, but they said I won't need them anymore. And I still have the certificate of physical examination I took this October, will it still be valid if I'm to apply for extending the resident permit in May 2018 later?
