Does anyone have a link to download the Passport renewal form?

My Wife is Chinese with PR in the UK got the proof from the UK Home Office. That she has not applied for UK citizenship £198 for this letter. Now i need to download the passport renewal form from the Chinese Embassy. Could anyone please help? Maybe send a link? Any help will be greatly appreciated!
