About:China Work Visa

My employer in China is applying the work permit for me (US Citizen).

They had sent me some forms (i.e., Application Form for Foreigner's Work Permit) to fill out. However, I don't see the " Physical Examination Record For Foreigner " form in the package.

(1) Can you tell me where I can download the "Physical Examination Record For Foreigner" Form?
(2) Do my documents (i.e., Diploma, License, Marriage Certificate, Certificate of Criminal Record, etc.) need to translate into Chinese first "before" I send the documents back to my employer in China ? How and where can I do that?
(3) Do I need to apply for Work visa (i.e., Z visa) before applying for Work permit?
(4) Are there commercial services companies provide this type of services in US? (i.e., visa application services company, etc.)

1. You can download the Physical Examination Record for Foreigner Form from this website.
2. Usually, those documents don’t need to be translated into Chinese.
3. After you get the work permit, you can apply for the work Z visa.
4. There are some agencies proving this kind of service.

(1) You'd better do the physical examination in China, instead of in your country.
(2) You'd better do the notarization of your Diploma and no-criminal record in your country, which is required when applying for a China work visa. As for the translation, it is not so urgent.
