Require Check List to obtain Q2 visa from Karachi Pakistan.

Dear Officer, One of my friend planning to apply Q2 type from Karachi Pakistan,earlier in 2014 he had obtained Q2 type and he traveled on that visa, now I need to know, is there any change in application form or in the relevant policy and can i use normal application form which we use for "M" and other visas and is there any application fee for Pakistan Nationals and also what is the processing time and could you kindly send me a check list of Q2 type, a favorable consideration will be highly appreciated, kindly reply soon, Thanks.
