China Visa Application in Osaka for Malaysian

I hold a Malaysia Passport. Currently, residing and working in Japan. I have a work permit. Coming December 2016, my friend and I plan to visit China (Tibet and Shanghai). I would like to know how can i apply for an entry permit?
1) Where can i get the application form? Which is the correct form to be filled?
2) Where can i pay the application fees? Direct the Osaka Consulate?
3) Do i need to make an appointment prior to my visits to the consulate? How to make a reservations?
4) What is the require documents? I read some Q&A above, so do i need to bring transportation arrangement and hotel booking record? Meaning i need to purchase all my travel plan tickets and hotel booking completed before i apply?
Thank you

1. Wei, you can download the application form at this website or at the official website of the embassy of PRC. You should fill out the application form V.2013.
2. You can pay the application fee directly at the Osaka consulate.
3. You don’t need to make an appointment in advance.
4. You need to prepare these documents: your passport and its copy, the application form, a passport-style photo, your valid entry permit for Japan, and proof transportation and accommodation arrangement. The proof of transportation and accommodation arrangement is necessary if you don’t have the invitation letter. You should purchase the air tickets and reserve the hotel before you apply.
