Portuguese applying for a China visa in Poland

Hi, I am a Portuguese living and working in Poland for 3 years (I have karta pobytu and PESEL). I will go to PRC as a tourist (L type).
Please correct me if I am wrong:
1) Where do I have to go to request the L type? Warsaw? Gdansk? (I live in Krakow)
2) How many times I need to go there?
3) How much will I pay for 1 entry? 150zl as I am inside the category "Other Countries"?
4) I don't have a recent passport photo as said on point 3. The picture for passports in Portugal are made on the same day using a photo+signature all-in-one machine on the office. Can I attach one of my recent photos or even take another one in 'passport-style'?

1) You should apply for it at the embassy of PRC in Warsaw.
2) You just need to go there twice: submitting your application and collect your passport
3) It costs 160 zl.
4) if your recent photo is passport-style (a recently-taken photo, bare-head, full face against a light background), it can be used. Otherwise, you should take another one in passport-style.
