Traveling with a minor

I am traveling to China with my sixteen year old daughter. Do I need to fill out a special approval form since she is a minor?

Lisa, you need to provide a notarized Parental Letter of Consent authorizing the travel from your spouse. Furthermore, except the approval letter, the minor also needs to provide the original and copy of her birth certificate or the relationship certificate as well as copies of her parents’ passports or ID cards and other basic documents.
