About:Chinese Embassy in Trinidad and Tobago

One Form need to fill for both Husband and wife or separate forms?

I and my wife will be travelling to China to do our annual purchasing for our company M.E.T. Wholesale Ltd.
DO we each filled out a form or would one form do for both of us?
