Regarding filling out section 2.5

My spouse and I are going on a 12 day tour with an agency. Do we still need to fill out section 2.5 and enter each day with a residence and phone number? We will be staying in hotels and the last 3 days is a cruise. The tour agency has not answered my emails requesting detailed itinerary with that information.
