You need to apply for two separate entry permits to HK and mainland PRC.
For HK, you need to provide those documents containing copy of your passport, the application form and a passport-style photo, proof of financial support, flight itinerary, and proof of travel purpose to HK.
For mainland PRC, those documents are needed: your passport and copy of its data page, the application form attached with a recently-taken passport-sized photo, an invitation letter from local travel agency, “Invitation Letter for Tourist” from Chinese Tourist Bureau, and your travel itinerary, as well as recent 3-month bank statement.
And then submit the two applications to apply for the entry permits.
