Standby to Shanghai then full fare to Hong Kong

We will be flying standby from LAX to PVG, staying 3 days and flying full fare to Hong Kong. Is there a form or application needed to take to the "72 Hour Visa " counter in the PVG airport and where is it obtained?

Well, you don't need to fill any form. After you arrive at the airport, you can follow the direction signs to the special lane for the 72 hour visa-free transit and apply for it by showing your onward ticket, passport and Arrival/Departure Card .
