The required documents contain the application form and 2 recently-taken color photos, relevant materials to prove his Chinese nationality such as his birth certificate issued by Philippines NSO or the non-philippines nationality statement authenticated by the ministry of foreign affairs of Philippines (original and copy), as well as residence permit in Philippines.
It charges 1400 pesos.
He should apply for it at the consulate general of PRC in Cebu.
