You need to provide those documents containing the filled-out application form with a bareheaded color photo glued, your passport and copy of its data page, proof of your financial support, transportation arrangement such as flight itinerary, and your travel plan to HK. And then submit your application to the embassy of PRC or post your application to HK immigration department to apply for it.
