How to Get Affidavit of Lost in China

May we know how to process the Affidavit of Lost? SIRV Card in Philippines was lost in China, please let us know the procedure and any requirements?

Juliet, first, you need to go to the nearest police station to report your loss and get a document regarding the loss. Then, go to the Exit and Entry Administration Division of the municipal Police Security Bureau and apply for a loss certificate. After you get this document, you can go to the Philippines embassy and see if they can issue a new SIRV card for you.
