query for admission in masters on scholarship

Question

Dear Sir/Madam,I've completed my B.Sc in Biomedical Engineering and want to apply for Chinese Government scholarship for my masters.I am contacting my targeted universities for Pre-admission letter, but kindly can you tell me from where can i get the original form JW202 and JW 201?and can i fill the Foreigner Physical examination form by any local doctor or any specific doctor recommended by PRC couslate then attest it from  Ministry of Foreign Affairs Islamabad.kindly reply

Dear, after you are admitted by the university, it will get the JW 201 or 202 form for you.You should go to the designated hospitals to get the medical report. The hospitals include KRL Hospital, Chinese Acupuncture Centre, and G.C.C.Diagnostic Centre. The medical report don't need to be attested by the Ministry of Foreign Affairs.
