query for admission in masters on scholarship

Dear Sir/Madam,

I've completed my B.Sc in Biomedical Engineering and want to apply for Chinese Government scholarship for my masters.

I am contacting my targeted universities for Pre-admission letter, but kindly can you tell me from where can i get the original form JW202 and JW 201?

and can i fill the Foreigner Physical examination form by any local doctor or any specific doctor recommended by PRC couslate then attest it from Ministry of Foreign Affairs Islamabad.
