Carl, you need go to Chinese embasy in Washington DC. It's located at 2201 Wisconsin Avenue, N.W., Suite110, Washington, D.C. You need to provide the following required documents:
1. finished application form for Chinse passport
2. 4 recent six months passport photos in white background
3. original and copy of your passport
4. valid entry permit or resident permit in the USA
5. an explanation letter, in which it includes your basic information and reason why you ask for passport renewal

According to what I know, there is no Chinese embassy or consulate in Utah. People who live in Utah need to go to Chinese embassy in Washington DC. Not only you but also US citzens in Utah need to apply Chinese visas from Chinese embassy in Washington.
