Caro, in general, you needn't to get a medical certificate before obtaining the Z type. After you enter with the Z type, you need to apply for a temporary residence permit with the Z type. At that time, a physical examination record is needed. You can get it from a hospital designated by the exit and entry administration bureau in China.1
