About:Chinese Embassy in Sri Lanka

do i have to fill separate visa application forms for each?

i'm visiting to china mainland with my wife and kids.

Sam, if your wife and kids have their own passports, then they need to fill the forms separately.You wife needs to fill the form on her own. If your kids are younger than 18 years old, you can help them fill the forms.
