You should first make an online appointment on the website of CVASC offices. During the time before the arranged application date, prepare the following documents: a valid passport, passport copy, filled-out application form, passport-type photo taken within six months, a declaration (can be downloded from the website) with your personal signature, as well as other supporting documents. Then submit the application on the due date.
