About:Visa Application for Children Born in US with Chinese Parents

Hi my daughter who is 15 will be traveling to PRC with her grandmother my daughter is a US Citizen what forms do I need to apply for her because I will not be there she will be going with her grandmother?

Since she doesn't reach 18 years old, you need to fill the appliation form and sign the name on her behalf. By the way, I think you and your husband need to write a letter that you both agree to let your daughter travel with her grandparent.
