Separate application form for my son who is 9 years old?

I am applying for a tourist L visa. I want to bring my son, who is 9 years old. Will I be needing to fill out a separate application form for him with separate supporting documents? Or just one application form will do? If one will do, shall I be entering his details in this question in this section in the form: "if someone else travels and shares the same passport with the applicant, please affix their photos and give their information below"

John, if you share a passport with your son, then you don’t need to provide a separate application form for him. You can enter his details in the section you mentioned.
