You should take your passport, a photocopy of the passport, a properly filled application form, a bare-headed photo taken within six months, the original and photocopy of your resident permit in Czech Republic, and other supporting documents that can justify your travel to China. Sometimes, the officer may ask you to submit the photocopies of your previous China entry permits as well.
