1. An application form attatched with your recent bareheaded photo.2. A valid passport and a photocopy of its info page.3. Travel itinerary, roundtrip tickets and hotel bookings.4. A personal statement.5. A certificate issued by your organization or the a bank statement in the latest six months.6. Other documents required by the embassy.
