Hey, you are required to provide the following documents and then submit your application to the CVASC branch that has the jurisdiction in your area.
1. passport with a validity of six months at least.
2. filled application form
3. a recent passpot photo
4. roundtrip air tickets and hotel reservation
5. a personal information letter
6. an employment proof or recent six months bank statement0
