Visa for mainland and Hong Kong in single trip

We wish to apply to visit Dongcheng District, Dongguan city for business. On the way back we wish to visit Hong Kong for 2-3 days. Is there any other forms for applying to visit HK (other than the one for China main land)?

Yes, you need to fill an application form and then apply a permit to visit HK, besides the China mainland one.
Besides the application, you still need to prepare your valid passport, a recent taken photo, round trip tickets to HK, hotel bookings, your travel plan and your bank statement (if any) to apply. You need to apply it in advance and then use it to enter HK.
