About:Chinese Embassies and Consulates

I am a permanent resident in Australia, but a Brazilian citizen.

Previously I have had to submit two application forms but I can't find the second application form anywhere.

Where do you make your application? Not all the Chinese embassies required two application forms.
If you applyin Australia, only one application form is required. Besides, you need to provide your permanent resident card for application.
