when to fill out part 5 on the visa application form

Question

I filled out the application form for my trip from the USA to Beijing, China but another person will bring my passport, application and other required documents to the application office for processing. Should I fill out Part 5 for the person who is bringing my application to the visa office and have her sign the form in 5.5? The instructions say that Part 5 is filled out if the form is COMPLETED by another person and this would NOT be the case, so I was going to leave Part 5 blank.  I want to be sure that it is okay to leave part 5 blank even though my colleague is bringing the application for processing.   Please advise. Thanks!

Please do not worry. It is OK to leave the Part 5 blank. Since you fill the form on your own, it is no need to fill the Part 5.
