I have questions on completing v 2013 appliction

How do I complete 1.5 -- Name in ethnic script. I am a citizen of the United States. Do I just put my name in English here?

How do I complete 1.11 --Local ID number? As a US citizen, we are not issued an Local ID card. However most of us use a drivers license for when id is requested or some other picture ID document.
