About:Chinese Consulate in Los Angeles

If I wanted to apply for a replacement Chinese passport what is the website on obtaining a form and

filling it out and then turning it in through the right place? What kind of documents do I need to bring to process my application? I'm helping my wife who misplaced her passport after coming from her hometown China a few months ago.

Hi, for Chinese passport renewal, Chinese citizen needs to visit the consulate in person. You need refer to the website of Chinese consulate directly but such information is only available in Chinese version. The application form can be downloaded from there as well.
Following documents are required:
1. Application form
2. 2 passport photo in meassure of 48mmx48mm
3. Original and copy of old passport or Chinese ID card, or Hukou Book to show her Chinese citizenship.
4. Original and copy of document showing her status in US
5. A loss report
