You needn't to prepare an import permit. First, get your dog injected with Rabies vaccine at the pet hospital. Second, ask the pet hospital to give you two certificates (health certificate and Rabies Shot certificate). Third, make an appointment with the USDA and get your dog's health certificate stamped. Last, ask the airlines if there are any other requirements.
Notice: Your dog's certificate should be done within 10 days before you fly to China. The Rabies Shot Certificate is valid for 3 to 6 months. Please make sure that it is still valid when your pet comes to China. When your pet arrives in China, it might be isolated for 30 days. It costs around CNY 70 per day.
If you do not fly with your dog, you'd better find a pet transfer company. They do all the jobs and transfer your dog to China. You needn't worry about anything.
