About:Requirements for Permanent Residence Permit

Certificate of Health for a foreigner applying for permanent resident permit as an outstanding alien

I'm currently employed in a Chinese company and my visa expires August 2014. I will be applying for a permanent resident permit this month. My questions are: 1. How to apply for medical certificate if I change it from working to a permanent resident permit? 2. My contributions to PRC are on education aspect where I provided free services and voluntary works in helping the country in improving the education system and giving financial help to the poor. I paid taxes while working in international schools.I have recommendation letters from most of my former and current students, co workers, friends and their families who benefited form my services and relationships. Can I be qualified to apply for a permanent resident permit? By the way, I've been in Shenyang for 12 years teaching english and an international education consultant and trainer for free to private institutions and students who are going overseas. Hi, it is very difficult to get this approved because there are strict restrictions and only few number of foriegners can get it successfully. Anyway, you are advised to visit your local PSB for details.
As I know, the most important document required is the original and photocopy of the Confirmation Form for Foreigners Permanent Residence Status issued by the Ministry of Public Security of China.
