Application form - China Travel Document

Can anyone direct me to the application form(s) "to download" for a Can anyone direct me to the application form(s) "to download" for a China Travel Document. Please and thanks.

You can visit the website of Chinese consulate in Los Angeles and download from "Document Download" in the Passport and Visa column.

Thank you. I have been there. I don't see a "Document Download" option. There is a "Download Forms" option. In selecting it, there are forms to download, none of which are for a "Travel Document" application. any ideas? Am I doing something wrong?
