You need to prepare a letter of declaration. If you are divorced, you are required to provide the divorce certificate (both copy and original version). If your ex-spouse are dead, you need to provide the death certificate.

Please be noted that you should go to apply in person and sign your signature on the declaration letter in the presence of the officers of the embassy. The certificate is usually valid for six months.
