Irish-passport application

I have Irish nationality and I live in ireland. I got married with my wife[she is chinese] in China. Last month my baby was born in China,now I want to apply Irish-passport for my new-born baby and let my wife cometo Ireland to live with me together. Could you tell me what documents I will need to prepare and how can I get the application-forms? Thanks a lot.
