About:Chinese Embassy in Canada

Which form do I use for a Tourist Visa - I am a Canadian Citizen

We are taking a cruise this fall and making 3 stops in China - Qingdao for the day, Dalian for the day and than staying in Beijing for 3 days before flying home. Which form do we use and do we apply for? that is One entry valid for 3 months from application or do we need one for Multiple entries?

You need to apply for a multiple entries L. The V2011A form is enough application.
