About:Chinese Embassy in Malaysia

Question on surname and given name on application form

Currently, I am applying china visa application. I am wondering what should I fill up for the part of surname and given name. As in passport, there is no section for surname and given name. I am confused. For example, name on passport is Mike Lim Hock Seng. What should I fill it on the form? Would appreciate your help on this. Thanks.

Hi, your surname should be Lim Hock Seng, and given name Mike.
