visa request for me and my daughter

I am travelling with my daughter to China for a week. I want to apply for a visa for her. She has taken her fathers surname not mine. What do I have to bring in order to be able to get it?

How old is your daughter? if she is minor under 18 year old, birth certificate is required, maybe her father shall execute a duly-notarized Parental Travel Permit. You'd better contact the consular office directly for confirmation of this.
