visa china shanghai

i am Pr singapore ,but i am holding vietnam passport ,so i need apply visa when i travel in china -shanghai ,i just want to cmf requirement document as below.thanks:)

Tourist/Fmily Visit Visa (L)

1. Original passport with at least two blank visa pages and the passport should be valid for at least six months.
2. One completed and truly filled Visa Application Form (Form V2011A) with a recent-taken passport photo stuck on it; if the applicant has a Chinese name, the Chinese name should be filled in.
3. Those who apply for tourism in China should provide round-trip airline tickets and hotel reservation; for family visit should provide certificate of relationship, round-trip airline tickets and hotel reservation.
4. Non-Singapore citizens shall provide documents that entitle them to reside for an extended period of time in the SPR, WP, EP, STP, DP.
