x visa medical exam

Hi, My name is Oybek and I've enrolled in Univerisity in Beijing.I have invitation letter and admission notice and PHYSICAL EXAMINATION FORM. I'm from Andijan and would like to know where I can fill the PHYSICAL EXAMINATION FORM??Do I have to fill it in a particular place in Tashkent or can I do it in central hospital of Andijan city???would you email me the reply, please.hope to hear soon,Sincerely,Oybek

You should call the consular office of the Embassy or go to the embassy directly to get the list of the authorized hospitals by the embassy, collect a form deom the embassy, and then go to one of the appointed hospital to do the exams, and the hospital will fill the form for you.
