X visa medical exam

Hi, My name is Oybek and I've enrolled in Univerisity in Beijing.
I have invitation letter and admission notice and PHYSICAL EXAMINATION FORM. I'm from Andijan and would like to know where I can fill the PHYSICAL EXAMINATION FORM??
Do I have to fill it in a particular place in Tashkent or can I do it in central hospital of Andijan city???
