May i know what document needed to apply for Z visa in malaysian china embassy ?

Currently i am in Xiamen and holding a multiple visa entry to china for one year due to i just relocate my assignment from Beijing to Xiamen, and it seem due to the different regulation i would need go out from china to re-apply for Z visa to do my work permit .
Need to know what document is needed for applying Z visa
can i use my medical report which i just did Xiamen China to apply my Z visa back in malaysia china embassy
