About:Chinese Embassy in Azerbaijan

where and how to get an official Physical examination report (in Baku)

Dear Madam or dear Sir,

I am recommendet to bring my Physical examination report to Beijing.
It would be a great support to advice me where I can find here in Baku a Doctor or a Hospital where from I can get this report with your full contentment.

So, you mean a physical examination report for work Z application, right? this physical exam form, guidelines and list of accredited hospitals are available at the visa counter in the embassy or consulate. Generally, state-owned big ones are ok.
