how to fill out the supplementary form - chinese visa

Hi there,I have completed the Chinese Visa Application Form (Tourist Visa) but am confused as to exactly what details I have to complete on the Supplementary Form, some help here would be greatly appreciated.

If you apply for the China visa in your home country, if you won't apply for China Z or X visa, if you won't share the same visa with another, no need to fill and submit the Supplementary Form V2011B.  If you do need to submit this form, just fill the blanks with your genuin information.
