You need to fill out Application Form V.2011A and Supplementary Application Form V.2011B if you apply tourist L here, your wife just need to fill out Visa Application Form V.2011A. The other documents are:1. Your US passport and your wife's passport 2. each one's passport photo, affix it on the application form V.2011A3. hotel reservation and flight tickets 4. the original and copy of your permanent residence permit
