Visa application for firmer Chinese visa holders (Consular Staff)

I'm a former staff of the Philippine Consulate in Hongkong from 2005 to 2008 and was issued a Chinese visa during my assignment in HK. I was also able to visit Shenzhen and Zhuhai during my official assignment to HK. I'm still working with the Philippine government and I'm planning to visit (tour) Beijing this September 2012 with my family. Since I will not be using my Official Passport (issued for official travels of government workers) but will be using the Unofficial or Personal Passport what documents do I need to submit together with my application for Chinese visa? I would appreciate very much your promp reply.

Since you have been to China before, it will be easier for you to apply for a China visa even with your ordinary passport. You have to submit the following documents:
1. Your passport with blank pages and at least 6 months validity left before expiration.
2. A truly and completely filled application form affixed with one passport-size or 2x2
colored photo, with white background, full-face front view, and no hat.
3. Previously used China visa in old or new passport
4. round-trip air tickets.
