You can apply for a China Crew visa with the following documents:
1. Your original passport having at least 6 months validity and two blank pages as well as a copy of the information page.
2. One completed China Visa Application Form .
3. one recent photo of 2*2 square inch (black & white or color is acceptable) glued or stapled on the application form.
4. Formal application letter issued by the airline or shipping company;
5. The original and photocopy of the employee ID of the applicant.

With the visa, you may stay in China for 7-10 days for each entry.

The application form can be found on any official website of Chinese embassy and you can even find it on this website.
For the application letter, you can contact the airline compay and they will know the details. Basically, it includes your basic information, the purpose to go China and your departure time. The letter is to proof that you are an employee of their company.
